An exciting opportunity has arisen for a Team Supervisor within the Short Term Support Service covering the Southeast areathe post is based at Blyth Community Hospital. 

The position is full time (37 hours), Permanent, NCC Single Status, Band 5 salary (£26,421 - £28,770), based at Blyth Community Hospital. 
The Team Supervisor hours will be between the parameters of 08:30 - 17:00 Monday – Friday, not including bank holidays. 

The main aim of the short term support service is to provide timely care interventions to adults living in their own homes.

The role of the job: 
· A Team Supervisor will line manage a team of Reablement Workers and have a case load of clients. 
· On a daily basis the Team Supervisor will Manage staff rotas to co-ordinate the work of the team 
· Work collaboratively with referring professionals and team colleagues to develop effective team practice and ensure continuity in accordance with service standards. 
· Ability to form positive working relationships with Clients and colleagues. 
· Ability to work and follow clearly defined individual ‘Service User Plans’ including client, moving and handling and home risk assessments. 
· Able to write detailed clear instructions, case notes and reports including completion of electronic Client baseline assessments. 
· Listens, consults others and communicates clearly following written or verbal instructions. 
· Understands the need to respect confidentiality, privacy, dignity, independence, choice, rights and fulfilment of service users. 
· Adhere to and maintain Health & Safety of self, staff and others. 
· Follows policies and procedures, guidance, CQC Standards and Code of Conduct, whilst keeping to schedules. 
· Able to work unsupervised and as part of a team with Management support readily available when needed. 

Requirements of the job: 
· A good general education demonstrating literacy and numeracy. 
· Good General IT skills 
· NVQ Level 3 in Care. 
· Has a valid UK driving license (expenses will be paid in accordance with the organisations Single Status agreement rates) 
• Able to satisfy DBS requirements
