Team Supervisor with the North Short Term Support Service (STSS) – Permanent, Band 5 Post


An exciting opportunity has arisen for a Team Supervisor to work with the Short Term Support Service within the Alnwick area of Northumberland.   

About us: Our Short Term Support Service provides re-ablement and rehabilitation to adults living in their own homes to help them regain their independence after an illness, hospital discharge, or to avoid unnecessary hospital admissions. We also provide a bridging service for people who are waiting for a long-term package of care. The service includes re-ablement and therapy staff working together to promote independence in daily living tasks, including the provision of equipment and rehabilitation programmes. Our service is rated Outstanding by the Care Quality Commission.  

 
We are looking for a forward-thinking dynamic person who on a day-to-day basis will: 

  • Ensure confidentiality is maintained, and that each patient is treated with dignity and respect 
  • Effectively manage staff – including  supervisions, PDRs, authorisation of travel expenses, annual leave etc 
  • Effectively manage  CM2000  (electronic rostering and digital information system)  
  • Complete and follow Risk Assessments
  • Safely manage Care plans – including implementation, evaluation and review, promote independence  
  • Work in collaboration with clients and others involved in their care to ensure clients reach their potential and achieve best outcomes
  • Ensure Safe Administration of Medication – Follow guidelines, policy and procedures, and ensure accurate and timely recording; completion of audits
  • Carryout community visits to clients’ homes to assess/review re-ablement needs
  • Ensure robust documentation which is accurate and timely  
  • Comply with all relevant legislation and governance
  • Demonstrate effective time management
  • Complete training relevant to the Team Supervisor post incl competency to fit therapy aids/equipment, and facilitate training for staff as appropriate
  • Demonstrate excellent communication skills, and an ability to interact and engage with colleagues in a positive way to promote good team working


The working pattern is 08:30 - 17:00 Monday – Friday, not including bank holidays.

Requirements:

  • It is essential that the successful candidate can fully meet the transport requirements of the post, for which expenses will be paid in accordance with the organisations Single Status agreement rates. 
  • NVQ 3/equivalent or willingness to work towards an equivalent qualification.
  • Good general IT skills
