Team Supervisor – The Short Term Support Service (STSS) – Permanent, Band 5 Post Based in Hexham Northumberland.
An exciting opportunity has arisen for a Team Supervisor within the Short Term Support Service within the West area of Northumberland.
About us: Our Short Term Support Service provides reablement and rehabilitation to adults living in their own homes to help them regain their independence after an illness, hospital discharge, or to avoid unnecessary hospital admissions. We also provide a bridging service for people who are waiting for a long-term package of care. The service includes reablement and therapy staff working together to promote independence in daily living tasks, including the provision of equipment and rehabilitation programmes. Our service is rated Outstanding by the Care Quality Commission.
We are looking for a forward-thinking dynamic person whom on a day-to-day basis whom will;
• Ensure that confidentiality is maintained, each patient is treated with dignity and respect
• Management of staff - supervisions, PDRs, travel expenses etc
• Manage CM2000 – Rota
• Risk assessments – Develop and follow
• Care plan – plan, implement and review, promote independence
• Signposting onto other professionals or services
• Medication – Follow guidelines, policy and procedures, audits etc. Documentation – Accurate and timely
• Compliance with relevant legislation
• Manage caseload of clients
• Competencies – Therapy equipment
• Training – relevant to the Team Supervisor post and facilitate training
• Complete initial visits and home assessments
• Interact and engage with colleagues.
The working pattern is 08:30 - 17:00 Monday – Friday, not including bank holidays.

Requirements:

It is essential that the successful candidate can fully meet the transport requirements of the post, for which expenses will be paid in accordance with the organisations Single Status agreement rates.

NVQ 3/equivalent or willingness to work towards an equivalent qualification.
