Northumberland Adolescent Service are looking for a motivated and passionate practitioner to take on an opportunity within their 14+ Social Care Team.
The successful candidate will work with young people who are leaving care and moving on to independent / semi-independent living.
We are looking for a practitioner who has direct experience of working within a multi-agency setting to work to address young people’s support needs as they transition to adulthood.
You will join an experienced and committed team of social workers, leaving care support workers and a social work support assistant who all share the same ethos around achieving the best possible outcomes for young people.
You will carry a manageable caseload of young people and also co-work cases alongside social workers to support a smooth transition to leaving care.
The 14+ Team is co-located within Northumberland Adolescent Service. This is a specialist service developed to provide tailored support specific to adolescents. It is composed of a variety of services including an accommodation service, substance misuse service, youth justice service, exploitation team and the participation team
